Every fact in our corpus carries the physician who verified it and the date they did. A claim is not a fact here until a clinician puts their name to it.

Why this exists

General-purpose models are confident about medicine whether or not they are right.

That is tolerable when the subject is trivia. It is not tolerable when a patient, a clinician or a regulator is on the other end. The gap is not reasoning — it is the absence of a source anyone is accountable for.
